If you get a chance to browse thru this ICD-9 Coding Book, please do so. Because you will see that Carol Buck has inserted paragraphs of the Coding Guidelines within the Tabular Index -- you don't have to keep flipping back to the front of the book to check on the guidelines. The pages are very white which seems to make the black font of the text jump out at you - which is great when you are reading the Index. This book is chock-full of definitions, color diagrams and photos. I wish I had this book when I started -- a wonderful resource for experienced or beginning coders. A great investment !
